Care Management at North Valley Hospital
   

If you or a loved one is admitted to the hospital, it can be an overwhelming experience. You are forced to deal with insurance companies, financial stresses, the lack of information, and a variety of people and systems that may seem to be speaking a foreign language. There is medical information that is new and unknown, and you are faced with decisions that can have major consequences.

How does a person navigate through this experience? At North Valley Hospital, there are designated nurses who serve as guides to assist people all along the way. The position arose from consumer requests to have assistance with arranging homegoing and transfers to other facilities.
 
 
   
As health care has become more complex, the care management role has expanded so that patients and families now have a guide who can:
  • Provide information on financial resources.
  • Outline options for care within North Valley Hospital and within the community.
  • Arrange follow-up care at home.
  • Assist with transfers to referral hospitals.
  • Advocate with insurance programs and meet the requirements for insurance certification and review.
  • Coordinate long-term care and assisted living.
